The Tele-Monitoring Team gives us the ability to monitor patient with various co-morbidities 24 hrs a day by a skilled nurses to ensure better outcomes and less re-hospitalization. This is only part of the available nursing services. 

Care Unlimited is using the HomMed system exclusively in this area. The system performs a daily home monitoring assessment of weight, heart rate, SpO2, blood pressure, and temperature. In the coming weeks we will also have the capabilities to monitor glucose, lung function, and PT/INR. The device uses verbal prompts to walk the patient through collecting their vital signs every day at a set time - it takes about 3 minutes to complete. 

The readings are then transmitted to a central monitoring station where our clinical staff can review the data daily. Alert parameters can be set for each patient, so that if a reading is out of range our clinical staff is notified and intervention can take place. The patient's data can also be trended, in graphic and/or tabular form, for review and assessment.
